NDIS services

Concept Care’s proactive client services team has diligently matched more than 300 clients with the right support workers who align with their beliefs, interests, and cultural preferences.

They help clients with:

  • Daily living activities
  • Personal care (including high care)
  • Domestic assistance
  • Meal preparation
  • Yard and garden maintenance
  • Community access, travel and transport
  • Respite/short-term accommodation (STA), 24-hour care
  • Day programs (recreational activities) and
  • Companionship, skill building (including budgeting, travel training etc.) and support coordination.

Home care packages

The Home Care Package scheme is a government subsidised service accessible by older Australians (65 years and above) to support them living independently at home.

Depending on the approved package level, a range of supports can be accessed, such as:

  • Personal services (bathing, showering, grooming, dressing, toileting, etc)
  • Home, garden and yard maintenance
  • Assistance with meal preparation, nutrition, hydration and diet
  • Occupational therapy, physiotherapy, podiatry and case management and
  • Companionship, community access, transportation, and leisure activities.

Private care clients

Private in-home care can be tailored to meet individual needs. Even if Government subsidies can’t be accessed, a range of services are still offered by Concept Care.

These private in-home supports are self-funded, which enables clients to access services at any age for any length of time without any assessment required from the Government.

Respite

Short-Term Accommodation (STA) is a practical solution for clients who require temporary support out of home, allowing carers to have short-term breaks from their caring responsibilities. This support can be accessed at flexible intervals for up to 14 days at a time, dependent on individual needs and requirements.

STA supports are available as:

  • One-on-one short-term care
  • Group short-term options where participants are matched with other participants who have similar requirements
  • Overnight or weekend support where individual needs and requirements are aligned with STA facilities.

As an approved Voluntary Out of Home Care provider, Concept Care also provides overnight respite services for clients under the age of 18.

Holiday Support

Funding for short term accommodation falls under the core budget. The NDIS price guide defines short term accommodation as “Integrated support for self-care, accommodation, food and activities in a centre or group residence for short periods.”

Therefore, a portion of a participant’s funding may be used towards accommodation and recreational expenses, as well as bringing a support worker along.

Recreational programs

Concept Care’s recreational programs are designed for people of all abilities to participate and enjoy. 

Their workshops are designed to help individuals build and engage in social relationships, make personal connections, enhance concentration, confidence and self-esteem, reduce stress or anxiety and ultimately help individuals in developing skills that will support them in independent and community living.

Support Coordination

As a support coordinator, Concept Care connects participants with an NDIS plan to available support and services within the community. Staff will:

  • Help with understanding NDIS plans
  • Manage resources to get the most out of each plan
  • Negotiate services and their pricing
  • Arrange assessments that are critical in determining the level of support and funding needed
  • Collaborate with all the people in a support network
  • Monitor the progress and goals of each plan
  • Assist with review meeting preparation
