Blossom Community Care is a registered National Disability Insurance Scheme (NDIS) provider offering an extensive range of support and services to help participants reach their goals.

Blossom Community Care’s knowledgeable and experienced Support Coordinators guide participants through the NDIS process, from application to implementation and management, while their Plan Managers continue to work with participants to manage their plan.

  • Dressing on a daily basis
  • Grooming
  • Hygiene (including oral hygiene)
  • Bathing
  • Toileting (including menstrual care)
  • Mobility (including getting in and out of bed or on and off the toilet)

  • Basic First Aid

Blossom Community Care provides travel and transport services so participants can live life fully. Participants can choose when and for what reason they require travel or transport support, from medical appointments to community access.

At Blossom Community Care, Community Access is about enriching lives, fostering relationships and building independence.

There are several activities, events, workshops and more that empower the people to take control of their own lives. This might include social clubs, after school and holiday programs, cooking programs and even photography and swimming.

There are also a lot of workshops that aim to instil a sense of independence in participants. These include:

  • Developing relationships and social skills
  • Money management
  • Bill payment
  • Employment skill
  • Fitness training
  • Shopping
  • Attending appointments

Blossom is proud to conduct regular group activities and events so participants can socialise. Many of the organisation’s group activities take place at their head office in Truganina, such as art, cooking, exercise, yoga and sport workshops.

They also organise fun excursions and day trips around Victoria.
